Chapter guide
A project usually moves through understanding the work, framing the problem, co-design, testing, use and handover. Follow that sequence where it helps, but revisit a problem or solution when new information invalidates it.
Separate four record types. Facts need sources and dates. Assumptions need tests and disproof criteria. Decisions need options, owners and review conditions. Outcomes need observations and measurement definitions. Link records by ID instead of copying the same material.
A smooth demo, agreement in a meeting and complete documents still need scrutiny. Were only easy examples selected? Has an owner accepted the consequences? Did the meeting produce a concrete decision?
