System problems include slow responses, errors, unauthorized access and failed recovery. Problems at work include a return to old tools, lost management support and extra tasks with no owner. Neither kind is guaranteed to trigger an alert.

Report both system performance and actual use. After a successful demo, check real tasks, exceptions, permissions and support. An improvement in one metric does not show that every part of the work has improved.

Look for cases that challenge your view: who has not used the system, which shift struggles, and who does more work than before? If you find no problems, check whether you only observed the easiest situations.
